This page is for incoming student-athletes who will be participating in any of our five NJCAA sanctioned sports for the 2011-2012 school year.

All student-athletes must complete the following documents in order to participate in athletics at State College of Florida,Manatee-Sarasota:

1) Medical Packet (must sign and return) Medical Documents Packet

• Physical Assessment (within 1 yr.)
• Health Insurance Form (with copies of insurance card front & back)
• Sports Medicine Card & Authorization to Release Medical Info
• Athletic Health History Form
• Medical Consent & Liability Waiver
• Concussion Information Sheet
• Drug Consent Form

2) Athletic Department Packet (must sign and return) Student Athlete Packet

• FERPA form
• FAFSA form
• Buckley Documents (6 pages)
• Scholarship Agreement form (Letter of Intent Scholarship Athletes Only)

3) Drug Education Packet (informational packet only, no need to sign) Drug Education Packet

4) Student Athlete Handbook (will read and sign at the Handbook Workshop)

5) Housing Agreement (all athletes should read, only athletes residing in SCF housing must sign) Housing Agreement
