Attention-Deficit / Hyperactivity Disorder (ADD/ ADHD)

Definition    ADD/ADHD is a medically defined neurological disorder.  Manifested by difficulties in attention and concentration, impulse control, and activity level.  Cannot be cured but must be managed.

Documentation Guidelines    Documentation from a medical doctor or other qualified professional indicating the diagnosis and DSM-IV code including a statement how the ADD/ADHD affects the student in a classroom or test taking situation (short-term memory, concentration, processing deficits, auditory, speed).
