2021 Health Insurance Options
State College of Florida offers a choice of three different medical plans from Florida Blue:
Plan 1: PPO Blue Options 5772 High Deductible Plan
This is a preferred patient organization (PPO) plan that allows you to select health care providers from within or outside of a provider network. You will receive the greatest benefits when you obtain services from providers within Network Blue. This plan is offered with no cost to the employee, has a higher deductible, and has lower premiums for dependents.

Plan 2: HMO Blue Care 55 Plan
This is a managed care plan (HMO) through Health Options, Inc. You must select a primary care physician at the time of enrollment. All medical care must be coordinated through your primary care physician.

This is a preferred patient organization (PPO) plan that allows you to select health care providers from within or outside of a provider network. You will receive the greatest benefits when you obtain services from providers within Network Blue.

** SCF gives you the choice of 3 medical insurance plans from Florida Blue. SCF also offers employees the chance to reduce their health insurance premium if they complete a Health Risk Assessment (HRA) at the time of their new hire. If the employee completes the HRA, they have the opportunity to apply a $30 monthly credit towards their health insurance. Applying this credit towards the health insurance rates are noted in the second rates reflected in the chart above. |

BlueOptions, BlueCare - If you are in the PPO 1 or PPO 3 plan, select Plan BlueOptions. If you are in the HMO, select Plan BlueCare. HMO participants must select a doctor, use the NPI# when enrolling.
If you have medical insurance already and do not want to take one of the three plans offered by State College of Florida, the DV Plan (Dental and Vision) will be provided for you at no charge.
Traveling?
If you will be traveling outside the state of Florida, we encourage you to contact FL Blue to notify them. The Blue Card Programis a program that can help you in an emergency if you need medical assistance while traveling (and have one of the PPO plans). If you have the HMO plan, please see the BlueCard Program or Away From Home Care Program.
Prescription Coverage
Prescription coverage is combined with our medical insurance through Florida Blue. We offer a discounted prescription program through Prime Mail in which you receive a three month supply of prescriptions for the cost of a two month supply. That's a one month supply savings! Florida Blue provides a medications listing for those members in the plan who are curious as to what tier their prescriptions may be.
