Please contact the Financial Aid Services office staff at SCF Bradenton or SCF Venice to determine if any documents are required.

Verification Forms (Only Submit a Form if Requested)

If you have been selected for FAFSA Verification, the most commonly required forms are available below. If you receive an email or have a Financial Aid Verification Requirement listed on your MySCF account that is not below, please request the form from the Financial Aid Office. Contact options are on the left of this webpage. Verification of identity requires students to present a non-expired/current government-issued ID (Examples: State Driver’s License, State ID Card, US Passport, Permanent Resident Card) when completing this form in the Financial Aid Office or before a licensed public notary, and the original document with “wet signatures” must be submitted. Other verification documents can be returned via email/fax/mail/help-request.

Adjustment Requests/Petitions:

  1. Request a decrease in your Federal Direct Stafford Loans
  2. Request a cancellation of your Federal Direct Stafford Loans
  3. Request a decrease or cancellation of your Federal Work Study
  4. Declining Financial Aid Award
  5. Title IV Authorization
